An investigation at Princess Margaret Hospital has concluded that inadequate communication and a lack of reminders were to blame for the death of a patient following a liver biopsy in January.

The 60-year-old man underwent the procedure two hours after he was injected with an anticoagulant. But there should have been a 24-hour gap between the medication and the procedure.

He suffered bleeding from his liver and died a few days later.

The hospital says it accepts the panel's recommendations to boost communication between its clinical teams, develop a preparation guide for bedside liver biopsy and revise its Bedside Procedure Safety Checklist.

The case has been referred to the Coroner's Court.
